The Clinical Neuroscience Institute (CNSI) provides comprehensive care to people with neurologic conditions. Regardless of which location you visit, our coordinated care provides an excellent experience. The CNSI office you choose will provide detailed information specific to your care.

Appointment/Procedure Preparation

To make your appointment, please call and you will be advised how to prepare for your visit or procedure.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

Please be sure to bring your medications (in their original containers) or a list of the medications you take. Also bring your insurance card(s) and a photo ID. Be prepared to pay any co-pays, deductibles or co-insurance at the time of your visit.

Please verify your insurance coverage before your appointment and check our list of major insurance plans accepted by most providers at Premier Physician Network.
