Appointment/Procedure Preparation

If you need to cancel your appointment, please do so 24 hours before your appointment time. We understand that your time is valuable, and we’ll make our best effort to stick to scheduled appointment times. However, emergencies may cause delays.

Please make sure you bring:

  • Your photo ID
  • Your insurance card 
  • A list of all your current medications  
  • Your co-pay

What to Expect

At your first appointment, your physician and/or provider will give you a comprehensive examination. 

We will review all of your current prescription medications and issue necessary refills. Our staff will also review our prescription refill policy during your appointment. Depending on the nature of the visit, your appointment may take about 30 minutes. We encourage you to ask questions or to discuss any concerns you may have.

Medical records are completely confidential and will not be released without your consent. Please update us of any changes in address, telephone number, or insurance coverage as they occur.  

Patients should be prepared to pay co-pays, deductibles, or co-insurances at the time of their visit. Patients who are unable to supply their insurance card(s) and/or payment will be asked to reschedule.

After Your Visit or Procedure

Your physician will discuss your specific care instructions with you during your appointment. Follow-up instructions will vary depending on the service performed. If you need a return appointment, we will gladly schedule one at the end of your visit.

If you need any testing done, we will provide instructions to you and schedule your follow-up appointment. You will be contacted when your test results are available.

Walk-In Clinic

We have a daily walk-in clinic from 12:30 - 1:30 pm staffed by our providers.  Please be advised that you will be required to pay an office co-pay for these visits. At these visits, you can get blood pressure checks, receive treatment for swelling, discuss concerns about medication changes, and complete forms.  

Billing and Insurance

Please come prepared to pay your co-pay at the time of your appointment.

Please verify your insurance coverage before your appointment. Find a list of the major insurance plans accepted by most Premier Physician Network practices online. Middletown Cardiology Associates accepts most of these insurance plans.
